    This Privacy Collection Statement provides information about Virtual PAC’s collection, use and disclosure of personal and sensitive information for the purposes of Anaesthetists providing pre-operative services to patients. Renewable Anaesthesia Technologies Pty Ltd (ABN: 90 667 871 187) operates the Virtual Pre-Admission Clinic platform ("Virtual PAC") in accordance with all applicable Australian privacy laws.

     
    Definitions
     

    Personal Information

    Means information or an opinion about an identified individual, or an individual who is reasonably identifiable, whether the information or opinion is true or not, and whether the information or opinion is recorded in a material form or not.

     

    Sensitive Information

    Means information or an opinion about an individual that includes health information and health records and has a higher level of privacy protection.

     

    How does Virtual PAC collect personal information?

     

    Virtual PAC collects information about patients directly, where reasonable and practicable to do so. Otherwise, from other  authorised parties on your behalf such as parents/guardians or with your consent or other medical professionals or providers.

     

    Virtual PAC collects this information using a variety of means, including electronic or paper documents, emails and telephone calls and online tools (such as the Virtual PAC Platform).

     

    Purpose
     

    Virtual PAC collects Personal and Sensitive Information for the primary purpose of delivering the Virtual PAC Platform which facilitates communication and sharing of data between patients and their Anaesthetist and also necessary associated administrative activities.

     

    In particular, the Virtual PAC collects information for the following purposes:

    ·       to coordinate and enable communication between Anaesthetists and their patients/guardians including of patient Personal and Sensitive Information;

    ·       to communicate with third parties (including other medical practitioners, medical clinics, hospitals) where patients so consent;

    ·       to comply with necessary regulatory requirements, standards and reporting obligations;

    ·       for any other purposes that would be reasonably expected. 

     

    Virtual PAC may also use Personal and Sensitive Information for related secondary purposes, that you would reasonably expect, or as otherwise required or authorised by law.

     
    Failure to collect information
     

    If Personal Information is not collected, Virtual PAC may not be able to proceed with providing services. If you have concerns regarding any of the information that we collect or how it will be used, please contact us using the details below.

     
    Disclosure
     

    Virtual PAC will only disclose personal information to the extent reasonable or necessary to do so in the circumstances. By providing the Personal and Sensitive Information that Virtual PAC requests, you consent to Virtual PAC disclosing and providing that information to your Anaesthetist (i.e. the one who invited you to the platform) and their team.

     

    Additionally, the Virtual PAC Platform may let you choose to disclose your Personal and Sensitive Information to your other medical providers such other doctors and the hospital or clinic where your future procedure is performed within, as appropriate. If this is required, express electronic consent will be requested from you prior to any disclosure.

     

    Otherwise, Virtual PAC will not disclose this personal information to any third parties without your consent unless that disclosure is required or authorised by Australian law.

     

    Virtual PAC does not generally disclose personal information about an individual to a recipient outside Australia, and will not do so without:

    ·       obtaining the consent of the individual (in some cases this consent will be implied); or

    ·       otherwise complying with the Australian Privacy Principles or other applicable privacy legislation.

    Virtual PAC’s Privacy Policy includes information about how you may access your personal information or seek correction of your personal information. The Privacy Policy also includes information about how you can make a complaint about how your personal information has been handled.
